Part-Time Nurse Practitioner – Remote, Transitions of Care

Job Description:

We are seeking an experienced Nurse Practitioner (NP) for a part-time remote role focused on Transitions of Care. In this position, you will work closely with a team of experienced pharmacists to ensure continuity of care for residents as they transition across various care settings, with a primary focus on medication authorization and reconciliation.

Responsibilities:

• Receive medication handoffs from pharmacists and authorize medications as part of the care transition process
• Conduct medication reconciliation to ensure accuracy and continuity of care across transitions
• Collaborate with the pharmacy team to resolve any medication-related issues and provide appropriate follow-up care
• Ensure patients receive the correct medications at the appropriate times and settings
• Maintain accurate and up-to-date patient records in compliance with healthcare regulations
