Monday - Friday between 8am - 6pm and you may be required to work Bank Holidays & weekends on call on a rota basis

We are looking for a Care Liaison Advisor to join our team, where you will play a vital role in our company's success. In this role, you will closely collaborate with hospital discharge teams and care homes to source the best care packages for our clients.
This position offers a unique opportunity to work towards quick and safe discharge plans whilst supporting families during challenging times.
